Mouse suggestion.
I have the Razor Naga and love it. (though the forward/back buttons are in a hard to reach place. But the 12 extra buttons on the side are a big help. All buttons are configurable, with macros, set actions, etc. Plus it has profiles that can be triggered by application focus, so you can have a CAD set up and a Photoshop set up and it will automatically load/switch profiles as you go between the apps. Takes a bit to set up all the macros, but worth the time.

I use a Logitech G5 gaming mouse. Hideously ugly, but works well. 5 buttons, and tilt scroll wheel that I have programed to switch to Arrow / select and Dynamic Rotate. Logitech's software is notoriously worthless and buggy, but [URL="http://www.usboverdrive.com/USBOverdrive/Information.html"]USB Overdrive[/URL] works perfectly.

I use the logitech MX700 with six buttons plus right click and left click which are programmable.
